Dear 6th year students,
in connection with the current situation, please send me blue index in the form that you have to the address :

The envelope with document has to include the postscript: Blue Index.
Remember, that if you send blue index via post office, please do it with acknowledgment of receipt by registered mail.
There is also possibility that one student from 6th year will collect blue indexes and deliver them to ED, but it is obligatory to contact with me via e-mail till 03.06.2020 (Wednesday) and set a day when I can receive blue indexes.
Please do it as soon as possible, it’s important to receive diploma.
